Common variables causing under-payment of claims: - Is the practice's fee schedule set appropriately so maximum allowed reimbursement is received? - Was the reimbursement accurate based on the amount of units billed? - Were appropriate modifier codes used on CPT codes where reimbursement is usually deemed exclusive to another code/service given on the same date-of-service? - Did the patient pay the correct copay amount at the time-of-service?
